I am a Bangladeshi citizen, currently doing PhD in USA.
I do have only one name (viz. FAKHRUDDIN) in all of my certificates, passport, Visa. But in USA, all documents
require two names(First name and Last name)to be filled up. So far i have filled up my first name with FNU(FIRST NAME UNKNOWN)abbreviation. But i would like to change it. I like to add a first name to my existing name. For this what do i need to do? Will I have to change all my certificates, Passport for the changed name once i change it by affidavit? Or can i just keep the legal document of Affidavit and show it to anyone asking about my name's inconsistency?

To add a part of name you need an affidavit notarized by first class magistrate and after getting affidavit an add of such change in a newspaper. With this documents you have to apply for changing names in certificates.For changing name in passport you have to apply to the passport office with the same documents.
