I really dont like my birth name so I just want to change it to Raima Rafiq.
Is it possible for me to change it?
I am 15 yrs old and have already given my O level exams under the name ' Raima Islam'

To change name you have to swear an affidavit before the 1st class magistrate and publish an advertise of name changing in newspaper. You have to collect form from concerned authority of O level for changing name and have to submit paper cutting and affidavit.
