Alterating Name

My certificates of SSC, HSC and Honors as well as my NID and Passport includes my nickname along with my first and last names. I want to exclude my nickname. What is the procedure to do that? Do I have to do an affidavit? Because technically it is not a name changing, I will only exclude a portion. Or do I separately change the documents?

    You must have to do an affidavit and advertise in a popular newspaper. And then you have to collect form from education board, NID office and passport office for such correction.
